Make appointment / Refer

At HBCSHC, we are happy to see patients by referral or you can refer yourself. If you have a family doctor, it is generally better to get a referral from them so that we do not duplicate care and they receive a copy of your clinic note to ensure they are kept informed of your care.
For patients:
To book services, please call the clinic at 289-799-8771 or book online.

For appointments through April 10 at Jackson Square, click HERE.

You will be taken to the avaros website which will allow you to book an appointment if you have valid OHIP coverage.
For providers:
Medical providers may refer patients to our clinic using the form below or through standard paper or EMR referral, faxed to the Hamilton Birth Control & Sexual Health Clinic at 289-919-2505.
Please ensure all patient demographic information and referring medical provider's billing number, phone and fax numbers are included in the referral.

Please note: Referrals are accepted only for patients who either have a valid Ontario health card (or other province), IFH coverage, or UHIP.
Our office will contact both the referring provider and the patient with information about the patient's appointment.

For family physicians in a FHO/FHT, please take care to note the patient's rostered status. Providing a referral ensures that our doctors can bill a consult code which will not negate you. In-basket procedure codes may result in negation if they are not performed by a doctor with a focused practice designation.
